Medicare Private Contract Notice

Per federal regulations, Dr. Hamlin operates as a non-participating provider. 

To receive care at this practice, you must sign a private contract acknowledging that Medicare and Medigap will not reimburse our services. 

You retain the absolute right to receive care from providers who participate in the Medicare system elsewhere; however, our services remain outside of that reimbursement network.
